Student Health Forms
The Wayne County, Valley Health School Base Health Centers plans to be open 2 days a week at both WHS and SVHS.
WCS and Valley Health are hopeful this will help get the Well Child Exams, 7th and 12th grade immunizations, Sports Physicals, and other acute care visits done over the summer. We also have our Behavioral Health Therapist available over the summer.
Please have a legal guardian complete the necessary forms (linked above) before your first visit.

Spring Valley High School: Tuesday, 7:30 -3:30
Wayne High School: Wednesday, 7:30 to 3:30
Behavioral Health Hours
Wayne High School: Monday and Wednesday, 7:30 to 3:30
Spring Valley High School: Tuesday and Thursday, 7:30 – 3:30
